Valley Health Team, Inc. is a Federally Qualified Health Center (FQHC). We have staff available to assist patients in determining if they are eligible for a variety of health benefits coverage options. In many cases, FQHC staff can assist qualifying patients with enrollment.
FQHCs offer discounted medical services to qualified patients. Eligibility is based solely on income and family size. Please bring ONE of the following to the health center to determine if you are eligible:

The Comprehensive Perinatal Services Program (CPSP) provides a wide range of culturally competent services to Medi-Cal pregnant women, from conception through 60 days postpartum. In addition to standard obstetric services, women receive enhanced services in the areas of nutrition, psychosocial and health education. The Legislature enacted CPSP in 1984 in response to findings from the OB Access Project, which found that a comprehensive approach reduced both low birth weight rates and health care costs in women and infants. CPSP became a Medi-Cal benefit in 1987. Medi-Cal providers who provide services to pregnant women may apply to become a CPSP provider. Medi-Cal Managed Health Care Plans are required to provide access to CPSP-comparable services for pregnant Medi-Cal eligible recipients.
The Child Health and Disability Prevention (CHDP) is a preventive program that delivers periodic health assessments and services to low income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments are provided by enrolled private physicians, local health departments, community centers, managed care plans, and some local school districts.
Family Planning, Access, Care and Treatment (Family PACT) is a family planning program that helps eligible Californians avoid getting pregnant or causing a pregnancy when they are not ready.
